Vitamin B12 is the nutrient we get the most questions about at the counter, and for a simple reason: it's the only one whose intake depends entirely on what you eat, and the only one that can be chronically deficient without warning. Here's what it is, what it does—in the strict regulatory sense—where to find it, and what happens when you take too much or too little.
What is Vitamin B12?
Vitamin B12, or cobalamin, is a water-soluble vitamin built around a cobalt atom. It is the largest and most complex of the vitamins.
The crucial detail: no animal or plant produces it. It is produced only by bacteria and archaea. Animals contain it because their digestive flora produces it or because they have absorbed it; plants do not. This is why dietary B12 comes from animal sources, and not by chance or tradition.
Its absorption is a unique mechanism. Dietary B12 must be released by stomach acid, then bind to intrinsic factor, a protein secreted by the gastric mucosa, before being absorbed in the terminal ileum of the small intestine. This system is saturable: beyond a certain amount per intake, the excess passes through simple passive diffusion, with very low efficiency. This is exactly why you see high dosages on supplements, and we will come back to this below.
A final point of physiology, which makes deficiency insidious: hepatic B12 reserves are significant. An insufficient intake often takes years to manifest as clinical signs.

What does Vitamin B12 do—and what can legally be said about it?
Here, unlike many supplements, we are allowed to be precise, because B12 has authorized health claims under European Regulation 1924/2006. The exact wordings are:
- "Vitamin B12 contributes to normal energy-yielding metabolism"
- "Vitamin B12 contributes to the normal functioning of the nervous system"
- "Vitamin B12 contributes to normal homocysteine metabolism"
- "Vitamin B12 contributes to normal psychological function"
- "Vitamin B12 contributes to normal red blood cell formation"
- "Vitamin B12 contributes to the normal function of the immune system"
- "Vitamin B12 contributes to the reduction of tiredness and fatigue"
- "Vitamin B12 has a role in the process of cell division"
These formulations are conditional: they can only be used if the product provides at least a significant amount of B12 as defined by regulations.
What these phrases really mean
Pay close attention to the word "normal." These claims describe the nutrient's role in normal physiological function. They do not promise that taking B12 will make you less tired if you are not deficient. "Contributes to the reduction of tiredness and fatigue" means that B12 is necessary for energy metabolism, not that a tablet acts as a boost.
This is the nuance that 90% of sales pages omit, and it's also what explains frequent disappointment: a person with adequate intake who takes B12 to "feel energetic" usually feels absolutely nothing. Conversely, correcting a real deficiency changes a lot—but this is observed through a blood test, not a subjective feeling.
What not to expect from it
B12 is not a stimulant. It is not a weight loss product: nothing links it to body weight, and the weight loss aisle relies on other mechanisms. It is not a performance product: in this area, proteins, carbohydrates, and training are what work, as detailed in our article on daily protein intake. And it is not a treatment for persistent fatigue, which requires a diagnosis.
What foods provide vitamin B12?
Sources are exclusively animal-based, with a fairly clear hierarchy:
- Offal, especially calf, beef, or lamb liver: by far the richest foods, well above all others.
- Shellfish: clams, mussels, oysters, whelks.
- Fatty fish: mackerel, herring, sardine, salmon, tuna.
- Meats: beef, lamb, pork, poultry, in this approximate decreasing order.
- Eggs and dairy products: more modest but regular contributions, which matters for a vegetarian.

The case of plants and spirulina
A necessary clarification, because this is a costly mistake. Spirulina, algae, and unenriched yeast mainly contain cobalamin analogues, often called pseudo-vitamin B12, which the human body does not use and can even interfere with certain assays. Relying on spirulina to meet B12 needs is the best way to feel safe while developing a deficiency.
For a vegan diet, the only reliable sources are fortified foods (plant-based drinks, fortified yeasts) and supplements. This is not an opinion; it is a direct consequence of the fact that no plant synthesizes this vitamin. If you build your diet around plant proteins, B12 is the first point to address.

Does vitamin B12 have side effects?
It is one of the best-tolerated vitamins, and this should be stated as clearly as the risks mentioned elsewhere.
B12 is water-soluble: what the body does not use and store is excreted in the urine—hence the sometimes brighter coloration after intake, which is nothing to worry about. The Scientific Committee on Food, whose work has been adopted by EFSA, has not set a tolerable upper intake level for oral vitamin B12, due to the lack of established adverse effects at high intakes. This does not mean "without limit": it means that no toxicity threshold could be determined.
What is still reported:
- Acne-like skin rashes, described mostly with very high intakes and via injection. This is a documented effect, generally reversible upon discontinuation.
- Allergic reactions, rare, mainly related to cobalt or excipients.
- Interference with certain blood tests. A practical and often overlooked point: high-dose biotin (vitamin B8) distorts many immunological assays, including that of B12. If you are taking a B vitamin complex or a multivitamin, report it to the lab before a blood test.
Interactions to be aware of
Two families of medications reduce B12 absorption in the long term: metformin, and proton pump inhibitors or long-term antacids, which decrease gastric acidity necessary for vitamin release. These are well-described interactions. They do not mean that you should supplement on your own: they mean that the subject should be discussed with your doctor, who will decide on a blood test.
A specific contraindication deserves to be noted because it is little known: cyanocobalamin is not recommended in cases of Leber's hereditary optic neuropathy, where another form is preferred. This is a rare case, but it is a real case.
Who should monitor their intake and who should abstain?
B12 is one of the rare supplements where the question is not "who should abstain" but "who really needs to pay attention to it." Affected populations:
- Vegans, without exception. Supplementation is essential, not optional.
- Strict vegetarians with low egg and dairy consumption.
- People over 60, in whom decreased gastric acidity reduces dietary B12 absorption.
- After bariatric surgery or gastric or ileal resection: absorption is structurally compromised and medical follow-up is necessary.
- In cases of pernicious anemia, Crohn's disease, celiac disease: again, the doctor manages it.
- Under metformin or long-term antacids, as mentioned above.
Pregnancy and breastfeeding increase needs—EFSA considers a satisfactory intake of 4.5 µg per day during pregnancy and 5 µg during breastfeeding, compared to 4 µg for adults. This is not a contraindication; it's the opposite: it's a period when intake must be ensured, under medical supervision.

How to read the label of a B12 supplement?
The form. Four exist on the market: cyanocobalamin, methylcobalamin, adenosylcobalamin, hydroxocobalamin. Cyanocobalamin is the most stable and most studied; the body converts it into active forms. So-called active forms are often presented as superior, but no solid data supports the claim that they better correct a deficiency in a person whose metabolism functions normally. If a seller claims this, ask them what their basis is.
The dosage, and why it seems absurd. You will see products with 500 µg or 1,000 µg when the satisfactory intake is 4 µg per day. This is not marketing exaggeration: beyond the saturation of intrinsic factor, only a very small fraction passes through passive diffusion. A high single dose compensates for this low efficiency. However, a 1,000 µg product is not "250 times better" than a 4 µg product—it's a different absorption logic, not a different potency.
The percentage of nutrient reference values (NRVs). This must be on the label. A 40,000% NRV is neither a scandal nor a feat: see the previous paragraph.
What's around it. In a B complex, check the presence and dosage of B9 (folate): folate supplementation alone can correct anemia from B12 deficiency while allowing neurological damage to progress. This is another reason not to improvise and to get a blood test.
The galenic form. Sublingual tablets, drops, capsules, sprays: no robust data establishes the superiority of sublingual over conventional oral administration for B12. Choose what you will actually take every day.

Methylcobalamin or cyanocobalamin?
Cyanocobalamin is the best documented, most stable, and cheapest to produce; the body converts it. Methylcobalamin is an active form directly usable, but nothing demonstrates that it better corrects a deficiency in a person with normal metabolism. Both work.
Is spirulina enough for B12?
No, and this is a point on which we do not compromise at the counter. It mainly provides inactive analogues in humans. A vegan diet must include a supplement or fortified foods.
