Coenzyme Q10: Dangers, Side Effects, and Interactions
Coenzyme Q10 is an interesting case: a molecule whose biological role is perfectly described, yet for which Europe does not authorize any health claims. Both statements are true at the same time, and that's exactly what makes the subject confusing for the buyer. Here's what it is, what can be said about it, the real risks — which are modest — and the interactions, which are less so.
What is Coenzyme Q10?
Coenzyme Q10 is a fat-soluble molecule that your body produces itself and that participates in the transport of electrons in the mitochondrial respiratory chain, i.e., in the final stage of cellular energy production.
The detail behind the name: "Q" refers to the quinone function, and "10" to the number of isoprene units in its side chain — ten in humans, which distinguishes it from CoQ in other species. It exists in two forms, ubiquinone (oxidized form) and ubiquinol (reduced form), which the body constantly interconverts as needed. These are two states of the same molecule, not two different ingredients, even if marketing sometimes treats them as such.
Two practical points follow. First, it is fat-soluble: taken on an empty stomach, it is poorly absorbed. Second, it is mostly synthesized by the body; food — offal, fatty fish, meat — only provides a small part of it. This is a fundamental difference from a vitamin like B12, which the body cannot produce at all.
What does Coenzyme Q10 do, and what are we allowed to say about it?
Nothing, in a regulatory sense. No health claims are authorized in Europe for coenzyme Q10.
This is not a gap in the registry: the question has been investigated. In 2010, EFSA examined a set of applications relating to coenzyme Q10's contribution to normal energy metabolism, maintenance of normal blood pressure, protection of DNA, proteins, and lipids from oxidative damage, normal cognitive function, maintenance of normal cholesterol levels, and increased endurance capacity. None of these relationships were deemed established. All applications received an unfavorable opinion, and no claims have been authorized since.
It is important to understand what this means, and what it does not mean. It does not mean that coenzyme Q10 serves no purpose in the body — its mitochondrial role has been established for decades. It means that the available data have not shown that taking it as a supplement produces a measurable effect in a person whose synthesis is normal. A physiological role is not a supplementation effect: this is the distinction, and it is one that most sales pages erase.
What not to expect
CoQ10 is not a treatment for fatigue: the word "energy" here refers to a biochemical reaction, not a sensation. It is not a heart medication, and it never replaces a prescribed treatment. It is not a sports performance product — EFSA specifically rejected the application relating to endurance. And it is not an antidote to the effects of a statin, a point we will return to below.
Is Coenzyme Q10 dangerous?
Honestly, no, and it must be stated as clearly as risks are pointed out elsewhere. It is an endogenous molecule, very well tolerated in studies as well as over the counter, and we are not aware of any alert published by ANSES or EFSA concerning it.
What is still reported, generally mild and reversible upon discontinuation:
- Digestive disorders: nausea, heaviness, heartburn, especially at high doses or in a single dose.
- Headaches and dizziness, more rarely.
- Difficulty falling asleep when taken late. It is not a stimulant, but the feedback is frequent enough to justify taking it with lunch rather than dinner.
- Skin rashes, exceptional.
The real safety issue with this molecule is not toxicity. It is interaction.

Two situations constantly arise in stores, and these are the only two that truly justify asking the question before buying.
Vitamin K antagonist anticoagulants
Coenzyme Q10 has a structural similarity to vitamin K. A reduction in the anticoagulant effect of vitamin K antagonists in the presence of CoQ10 is described in the pharmacological literature, and this interaction is included in the interaction databases consulted by pharmacists. We are neither doctors nor pharmacists and we will not decide: if you are on an anticoagulant, this question should be asked of your doctor or pharmacist before any purchase, box in hand. A change in INR is not a detail.
Statins
This is the number one reason for purchasing in this department, and it deserves a precise answer. Statins inhibit an enzyme, HMG-CoA reductase, located upstream of a metabolic pathway common to cholesterol and coenzyme Q10. A well-documented consequence: circulating CoQ10 levels decrease under statin treatment.
But the question people are really asking is different: does taking CoQ10 reduce muscle pain attributed to statins? On this point, the results of trials are contradictory and no solid conclusion can be drawn. We can therefore neither promise it, nor tell you that it is useless. What we can say: muscle pain under statin treatment should be reported to the doctor who prescribed the treatment, because he or she can adjust the molecule or the dose. A supplement never replaces this conversation, and no one should stop a statin on their own initiative.
Other treatments
Interactions have been mentioned with certain antihypertensives and certain diabetes treatments, without the level of evidence allowing a rule to be drawn from them. The practical rule remains the same, and it applies to all supplements: under treatment, ask your pharmacist.
Who should refrain from taking CoQ10?
- People on vitamin K antagonist anticoagulants, unless explicitly advised by a doctor. This is the most concrete contraindication.
- Pregnant and breastfeeding women: safety data are insufficient in these situations, which is reason enough to abstain.
- Minors: no data to frame use, no reason to give it.
- Before surgery: report any supplementation to the medical team, as for omega-3s or any other supplement likely to interfere with hemostasis or anesthesia.
- Anyone on chronic treatment, until the pharmacist has given their opinion.
And a case we often see: the person who buys CoQ10 for persistent fatigue. We prefer to say it upfront — no claims exist in this area, and established fatigue first requires a blood test. The vitamin D, iron, and magnesium sections concern nutrients which, for their part, have authorized claims and reference values: we know what to dose and why.
How to read a Coenzyme Q10 label?
Ubiquinone or ubiquinol? Ubiquinol is sold at a higher price, presented as better absorbed because it corresponds to the reduced form. Available human data do not confirm that it performs better than ubiquinone at an equal dose once the question of formulation is taken into account. If a salesperson asserts this without reservation, ask for the source.
The galenic form, which matters more. A dry powder in a capsule is poorly absorbed; an oily suspension or a solubilized formula is better, because the molecule is fat-soluble. Check if the excipient contains an oil. This is the most determining criterion on the label, and curiously the least highlighted.
The quantity per capsule, not per "serving." Check how many capsules make up the announced daily dose. Market products are most often between 30 and 200 mg per dose; a prominently displayed figure sometimes corresponds to three capsules.
Origins. Food-grade CoQ10 is produced by fermentation, which yields the trans form, identical to that of the body, rather than by chemical synthesis. A label that specifies "obtained by fermentation" tells you something useful.
What's around it. Associations with vitamin E, selenium, or various extracts do not change the regulatory status of CoQ10. Above all, check the dosages of these additions: an ingredient present in trace amounts serves the list, not you.
Directions for use. A label that recommends taking it "with a meal containing fat" has understood its product. One that recommends taking it on an empty stomach, much less so.
