Turmeric: liver damage, side effects, and contraindications
Turmeric is one of the best-selling food supplements, and one of those on which French health authorities have been most cautious. This is not contradictory: the spice in a dish and the concentrated extract in a capsule raise very different questions. This page sorts it out, without dramatizing and without promising anything.
Turmeric, curcumin: what are we talking about?
Turmeric (Curcuma longa) is a rhizome belonging to the ginger family. Ground into powder, it yields the familiar yellow spice. Its active pigments, curcuminoids, constitute only a small portion of the dry rhizome—around 2 to 5%. Curcumin is the most prominent among them.
Hence a distinction that dictates everything else:
Powdered turmeric, the kind from your spice cabinet, provides a few tens of milligrams of curcuminoids per teaspoon. This is for culinary use, practiced for centuries.
An extract standardized to 95% curcuminoids, found in supplements, concentrates in a single capsule what several tens of grams of rhizome would represent. This is a change in order of magnitude, and it's precisely this change in scale that has prompted warnings.
The bioavailability problem, and what manufacturers have done about it
Curcumin has an embarrassing characteristic: it is very poorly absorbed. It is not very soluble in water, rapidly transformed by the liver and intestine, then eliminated, reaching the blood in very small quantities when taken as is.
The industry has therefore developed several workarounds, and it's important to know how to read them on a label:
Piperine, extracted from black pepper, often under the BioPerine® brand. This is the oldest and cheapest solution. A widely cited study from the 1990s reported a considerable increase in absorption with 20 mg of piperine. A rarely mentioned point of concern: piperine works by slowing down the metabolism of many substances—which also applies to certain medications.
Phytosomal forms, where curcumin is combined with phosphatidylcholine, and micellar forms, dispersed in a food-grade surfactant.
Extracts combined with turmeric essential oils, such as BCM-95®, where the rhizome's turmerones act as a carrier.
These forms work: they genuinely increase blood concentrations, sometimes by a significant factor. This also explains the precautionary stance of authorities, as we will see: a better-absorbed product is a product for which the dose must be re-evaluated.

What is the danger of turmeric?
We need to distinguish two levels, and this is where most pages miss the point.
At dietary doses, turmeric as a spice is not a major concern. Nobody recommends stopping adding it to a curry.
As a concentrated supplement, two points deserve your attention.
The first is the acceptable daily intake. Curcumin is also a food coloring (E100), as such evaluated by European authorities, who have set an ADI of 0 to 3 mg per kilogram of body weight per day. For a person weighing 70 kg, this represents approximately 210 mg of curcumin per day. Now consider a capsule of 95% standardized extract dosed at 500 mg: it alone provides 475 mg of curcuminoids, more than double the ADI. Many products on the market exceed this reference, and this is information that few labels display.
The second is hepatic. The ANSES (French Agency for Food, Environmental and Occupational Health & Safety) has received, via its nutrivigilance system, reports of liver damage associated with turmeric-based supplements, including several cases of hepatitis leading to hospitalization. These lesions are rare and idiosyncratic in nature—they occur in certain individuals without dose alone explaining it—but they were deemed serious enough to warrant official recommendations for caution, specifically targeting formulations with increased bioavailability, which were not covered by the initial evaluation of the coloring agent.
What this means concretely: this is not a product to take "just in case," continuously, at an arbitrary dose, and certainly not by combining several supplements that contain it without highlighting it.
Side effects: what is common, what should raise an alarm
The most frequent adverse effects are digestive: nausea, diarrhea, bloating, reflux, or heartburn, especially on an empty stomach and with high-dose forms. Taking it with a meal generally improves things, and a more modest dose too.
A temporary yellow discoloration of stools, fingers, or oral mucosa is harmless—it's a pigment. Contact allergies exist but remain rare.
Signs that require stopping and consulting a doctor are those of liver distress: unusual and persistent fatigue, loss of appetite, stubborn nausea, dark urine, pale stools, yellowing of the skin or whites of the eyes, itching. There is nothing to wait for or downplay in this case: stop the product and seek medical advice.
Who should not take it?
The list is longer than for most supplements, and it deserves to be read in its entirety.
Individuals with bile duct disorders—gallstones, obstruction, history—are explicitly targeted by caution recommendations. Turmeric stimulates gallbladder contraction, which is precisely what should not be provoked when a pathway is obstructed.
Individuals with known liver disease, or long-term treatment that is potentially hepatotoxic.
Individuals on anticoagulants or antiplatelet agents. An effect on platelet aggregation is described; combination should be discussed with a doctor. For the same reason, discontinuation is usually advised several days before surgery—report it to your anesthesiologist, just like any medication.
Individuals on treatment metabolized by the liver. Curcumin, and even more so piperine which often accompanies it, interferes with enzymes that break down many medications. The result can be an overdose or a loss of treatment efficacy, in both directions.
During chemotherapy, self-medication with concentrated plants is to be avoided without the agreement of the patient's medical team: interactions are possible and the stakes are disproportionate to the comfort sought.
Pregnancy and breastfeeding, due to lack of sufficient data, and minors, as these products are not formulated for them.
What can legally be said about turmeric?
Very little, and it's best to state it frankly. Health claims for plants in Europe are in a pending status: they have not been approved by the EFSA. In other words, no firm claims are authorized for curcumin regarding joints, digestion, inflammation, or anything else.
This doesn't mean that research is lacking—curcumin is one of the most studied plant molecules in the world, with thousands of publications. It means that this research, often conducted in vitro or on animal models, has not reached a sufficient level of proof in humans for a seller to be allowed to make a promise based on it. We adhere to this: we describe what a product contains, not what it would do.
How to choose, if one decides to take it
Look at the quantity of curcuminoids, not "turmeric." "500 mg of turmeric" and "500 mg of 95% standardized extract" represent a twenty to forty-fold difference.
Position yourself relative to 3 mg per kilo. Multiply your weight by 3: you get the benchmark in milligrams of curcuminoids. This is a benchmark, not a prohibition—but it gives you a measure of what you're taking.
The carrier is named. Piperine, phytosome, micellar, turmeric essential oils: each has its own logic, and an extract without a carrier will be poorly absorbed.
The duration is limited. Defined courses, with breaks, rather than continuous intake without an identified reason.
No blind accumulation. Check that your multivitamin or joint complexes do not already contain turmeric. With concentrated plants, it's the accumulation that causes problems, not the isolated product—the same reasoning we apply to ashwagandha or berberine.

Does turmeric thin the blood?
An effect on platelet aggregation is described, which justifies caution in cases of anticoagulant or antiplatelet treatment, and discontinuation before surgery. It is not an anticoagulant and does not replace any treatment.
Does turmeric raise blood pressure?
No direct effect on blood pressure has been established, neither upward nor downward. The real question, if you are being treated for hypertension, is the possible interaction with your medication via hepatic metabolism: discuss it with your doctor or pharmacist.
Does turmeric help with weight loss?
No, and no such claim is authorized. Weight loss relies on a calorie deficit; no plant extract replaces it.
How to consume turmeric for it to be effective?
Its poor absorption is the critical point: taken alone, the extract is very poorly absorbed. Hence the combinations with piperine, phytosomal forms, micellar forms, or with turmeric essential oils. Taking it with a meal containing a little fat also improves digestive tolerance.
Turmeric and cancer: what can be said?
Nothing that can be presented as a benefit. Curcumin is the subject of extensive preclinical research, but no claims are authorized, and no supplement treats or prevents cancer. During treatment, possible interactions require prior agreement from the medical team.
Does turmeric cause diarrhea?
This is one of the most common side effects at high doses, along with nausea and heartburn. Reducing the dose and taking the product with a meal generally resolves the problem.
Turmeric powder or capsules?
These are two different uses. Powder is a food, with a low curcuminoid content and very limited absorption. The extract capsule is a concentrated product, subject to the precautions described above. One is not a milder version of the other.
